Date of Incident-
We received a 911 call about a murdered dog, with fresh blood. I, as a Detective responded along with another detective. We then searched the barn which it's door broken. Few moments later after I went outside, Warren Sharpe requested for backup in the barn. I went back inside, with another patrol officer. Then we found out that someone is hiding in a room, above us. Minutes later, after changing rounds through the door, and Detective Sharpe shot, DAVID operators arrived and we decided to breach. I through in a flashbang, and tear gas inside. We breached and started to get fired upon from another room after clearing both right and left side. I started firing multiple rounds into the door but I then shortly got shot multiple times into the chest with a 12 Gauge Shotgun, whilst wearing a TYPE 4 Kevlar, and eventually lost all my blood. Shortly after other officers made sure that they neutralized the suspect, Portside revived me, told me to drop all my useful items and to /selfck. I shortly dropped all my items and did /selfck myself upon Portside's commands.

Steven Woods, as a SWAT operator, was wearing his Utility Uniform, his TYPE 4 Military Grade Kevlar, and multiple magazines filled with 5.56 rounds which could stop rounds like 9mm, .45, .22, .38, and parts of 12 Gauge.
With that being said, Steven Woods had a TYPE 4 Kevlar, which can stop up to 7.62 rounds, or even a bit bigger, from close range. So what about 12 gauge? Not to mention the magazines that are clipped into his plate which could slow the bullets if not stop them.
So, Steven Woods has a chance of surviving. He can suffer from injuries, as bullets might have gone through his gut, leg, arm. But I personally don't think it's enough for a CK.
Method of Death-
Multiple 12 gauge shots to the chest.
Have you attempted to resolve this already with the supervising administrator?-
